Waiver, Release and Agreement

LIKENESS WAIVER
I, on behalf of my child or ward (my “Camper”), hereby grant permission to Logan Ranch Camp, Inc. (“LRC”), the photographer, and parties designated by LRC, including clients, licensees, purchasers, agencies, and periodicals, the to obtain and use my child’s photograph, image, likeness, voice, video, and any other form of media (the “Likeness”), without compensation, as LRC sees fit for reproduction in LRC’s promotional materials of any medium, including video and Internet, for purposes of advertising, trade, exhibition, promotion, publicity, and editorial use. I, on behalf of my Camper, hereby grant LRC all ownership rights and the absolute and irrevocable right and permission to copyright, use, and publish the Likeness for LRC’s promotional materials. I, on behalf of my Camper, hereby release LRC from any and all claims, including claims for libel, slander, invasion of privacy, infringement of copyright or right of publicity, or any other claim related to the LRC’s use of the Likeness.

RELEASE
I hereby irrevocably and unconditionally release, waive, discharge, relinquish, and acquit LRC, Church at Camp, Team Logan, and their respective members, managers, employees, agents, officers, owners, the staff, and the volunteers (the “Providers”) from all damages and proceedings which arise from or relate to any of the following: (a) any participation in the Providers’ relief outreach referred to as “Church at the Camp’s Relief Outreach” (the “Camp”) or the gathering and participating in the meals, food, worship, service, games, and events hosted by the Providers or otherwise taking place at or at the direction of the Camp (the “Activities”) by my Camper, any guest, invitee, licensee, trespasser, or other participant, including illness, transmission of COVID-19 and any strains or variations thereof, injury, or death therefrom; (b) damage to or loss of property at the Camp or from participation in any Activities; (c) the condition of the Camp; (d) any instruction provided by any Provider, the staff, or the volunteers; or (e) strict liability and the sole, partial, contributory, comparative, concurrent, proportionate, or joint negligence of any of the Providers. I hereby agree to execute and return any Release and Indemnity Agreement, on behalf of myself and my Camper, which may, at any time, be requested or required by the Providers in exchange for my Camper’s participation in the Camp.

AGREEMENT
My Camper hereby agrees to follow all rules that the Providers, from time to time, establish for the Camp and its Activities, whether or not my Camper actually read such rules. Additionally, my Camper may not: (a) violate any applicable law or rule; or (b) engage in any unsafe, dangerous, illegal, or damaging activity. If my Camper fails to follow any rule, in addition to the Providers’ other rights, any Providers may, in such Provider’s sole discretion but without obligation, revoke any of LRC’s written or oral consent to my Camper’s limited, temporary use of and participation at the Camp and ban my Camper (temporarily or permanently) from the Camp or any participation in the Activities.
